A boy, 8 years old  with Body Weight 18 kg, came with a fever as a chief complain. The fever only has been 2 days.
The clinical sign only show the temperature is 38.5oC. Others is normal. The pharynx is only a little bit red, so I don’t expected he has pharyngitis.

After finished my pediatric phase, tomorrow I will begin my phase in ENT..
It is a bit refreshing because we only study in the hospital’s clinic, not in the ward.. So, no “follow-up the patients” thing..
The bothering part is the hospital for the ENT phase is locating at the next town..

Etiology
Corynebacterium Diphteriae, spread via droplet, air and food.
 
Clinical Signs
- Infected the mucous in the entrance of tract
- Making pseudomembrane in it
- Never be septic because it is only infected in port d’entree
- The patient can die because of asfixia and myocarditis
- One thing for sure, diphtery is so infectious! The patient must be isolized.

2 from 4 clinical signs must exist :
1. Sudden fever, 2-7 days
2. Hepatomegaly
3. Circulation disturbance
4. Haemorrhagic manifestation. It can be only just petechiae, but also spontaneus bleeding.
 
 
The laboratory examination must shown :
1. Thrombocytopenia. The thrombocyt is below 100.000/mm3
2. Haemoconsentrate. The hematocryt must raised >20%.
